* As a condition of participation in this retreat, the Recording Release, Liability Waiver, Medical Information, and Additional Information forms must be completed. We will email copies of these forms to you through DocuSign, and you can complete, sign, and submit the forms digitally. Alternatively, you can download and print the forms shown above and return the signed forms to the address located at the bottom of this page. The other documents are for your information and will assist in your preparation.
